In our latest SHIFT webinar, Lindsey Daugherty, head of community at Sage, brought together three industry leaders to share candid insights on how senior living communities can prepare for value-based care—without taking on more than their staff can handle. Phil Chuang (COO, HumanGood), Heather Tussing (President, Morning Pointe Senior Living), and Franke Elliott (Chief Strategy Officer, Bloom Healthcare) spoke about setting expectations, data collection, the impacts of value-based care, and more.

Define What Data Drives Value-Based Outcomes

Phil Chuang:

“I think it would be great to see at least states come together [to define value-based care in a way] that’s a little bit more consistent. In the absence of that, it’s about figuring out what we expect in terms of what we should know about our residents, and what information is going to be most helpful to provide care—or at the very least, referrals and connections to the care that they need.” 

Takeaway: Value-based care requires clarity. Define what information helps your staff deliver better care and coordinate effectively across providers.

Don’t Let the Payer Define the Plan

Franke Elliott:

“Today’s patient that's in Aetna might be tomorrow’s patient that’s in Humana [...] and it’s important that you’ve already captured their diagnoses, that you’ve built a care plan, that you’re managing them the same way—so that when they roll into these value-based care models, you’re not doing a lot of catch-up work.” 

Takeaway: Build the right care foundation now because, when payer models shift, you might not have time to catch up.

Value-Based Care Will Redefine Sales and External Referrals

Heather Tussing:

“It’s also going to affect how we think of sales for senior living. External business development and direct referrals from physicians—those high-conversion sources—could change completely. We need to think through that ahead of when we have to do it.”

Takeaway: Value-based care isn’t just a clinical shift, it’s a sales shift. Leaders need to rethink how referrals work before the old channels stop delivering.
